چکیده

Objectives. This study was performed to evaluate the effectiveness of education for trainees on the gross findings identified by conventional white-light endoscopy (CWE), the microvascular patterns identified by magnifying narrow-band imaging endoscopy (MNE), and the pit patterns identified by magnifying chromoendoscopy (MCE) in estimation of the invasion depth of colorectal tumors. Methods. A total of 420 endoscopic images of 35 colorectal tumors were used. Five trainees estimated the invasion depth of the tumors by reviewing the CWE images before education. Afterwards, the trainees estimated the invasion depth of the same tumors after brief education on CWE, MNE and MCE images, respectively. Results. The initial diagnostic accuracy for deep submucosal invasion before education and after education on CWE, MNE, and MCE findings was 54.3%, 55.4%, 67.4%, and 76.6%, respectively. The diagnostic accuracy increased significantly after MNE education (P = 0.028). The specificity for deep submucosal invasion before education and after education on CWE, MNE, and MCE findings was 47.9%, 45.7%, 65.0%, and 80.7%, respectively. The specificity increased significantly after MNE (P = 0.002) and MCE (P = 0.005) education. Conclusion. Brief education on microvascular pattern identification by MNE and pit pattern identification by MCE significantly improves trainees' estimations of the invasion depth of colorectal tumors.

Sub-classification of type VI pit patterns in colorectal tumors: relation to the depth of tumor invasion.

We analyzed pit patterns of colorectal tumors with magnifying colonoscopy, focusing on the relationship between the VI pattern sub-classification and the depth of tumor invasion. The VI pattern was divided into the well-demarcated and poorly demarcated subtypes. The percentage of tumors with a depth of invasion of over 1,000 microm was in the order of non-V types (type I-IV) < type VI < type VN...

Diagnostic Accuracy and Interobserver Agreement in Predicting the Submucosal Invasion of Colorectal Tumors Using Gross Findings, Pit Patterns, and Microvasculatures

BACKGROUND/AIMS Depth of invasion is one of the most important factors for establishing treatment strategy for colorectal tumors. METHODS Three blinded experts reviewed electronic photos and video clips of 33 early colorectal cancer-like lesions. They estimated the depth of invasion based on conventional white light endoscopy (CWE), magnifying chromoendoscopy (MCE), and magnifying narrow band...

Impact of clinical experience on type V pit pattern analysis using magnifying chromoendoscopy in early colorectal cancer: a cross-sectional interpretation test

BACKGROUND Although type V pit pattern analysis is effective in determining the invasion depth of early colorectal cancers, the clinical results may vary because findings are operator-dependent. This study aimed to assess the benefits of type V pit pattern analysis in estimating the invasion depth using magnifying chromoendoscopy compared to that with conventional colonoscopy. METHODS A cross...

Relationship between narrow-band imaging magnifying observation and pit pattern diagnosis in colorectal tumors.

BACKGROUND/AIM The aim of this study was to examine the relationship between narrow-band imaging (NBI) magnifying observation using the surface pattern as the main evaluation criterion and pit pattern diagnosis on the basis of magnifying observation using a dye in relation to the characteristics of colorectal tumors according to their morphologies. METHODS In this study, NBI observation and p...

Correlation of Narrow Band Imaging with Magnifying Colonoscopy and Histology in Colorectal Tumors

BACKGROUND/AIMS Narrow band imaging (NBI) is a new technique that uses optical filters for imaging of mucosal morphology. The aim of this study was to correlate findings of NBI with magnifying colonoscopy and histology for prediction of neoplastic colorectal lesion. METHODS Between September 2005 and December 2007, 107 colon polyps from 68 patients were detected by conventional colonoscopy an...
